This paper explores the reasons why case
management has been determined to be an important element for the cost-effective
improvement of
patient health. It looks at the objective of case
management, the defined roles of case managers, their functions and responsibilities, the various models of case management in health care systems and the differences in these models, and how case management affects patient welfare and economic variables. All of these aspects are explored with the intention of demonstrating the value of case management in the health care system.
