The aim of this study was to elucidate the effect of SMB resuscitation on the development of end-organ damage
caused by the ove-ractivity of RAS in animal models of pancreatitis. Sixty-four male SD rats were randomized into four groups (each n=16): sham operation group; SAP group; SMB group and 0.9%Lactate Ringer group (L.R); SAP was induced by intraductal injection of 1.5% sodium deoxycholate. In SMB group and L.R group in which Salvia miltiorrhiza Bunge and 0.9% Lactate Ringer were injected via the dorsum vein of penis after pancreatitis has been induced. The expressions of NF-κB in liver and ICAM-1 in intestine tissue was detected by immunohistochemical methods and the changes of plasm angiotensionⅡ(AngⅡ), LPS and TNF-α level were also measured at 6 and 24 h. Changes of intestine, lung and liver histopathology were investigated. Animals that received SMB showed less intestine, lung and liver damage than those resuscitated with normal saline. The levels of AngⅡ, TNF-α and LPS increased significantly at 6 and 24 h in SAP and L.R group after induction of SAP compared with normal group (P <0.05), but decreased significantly in the SMB group compared with L.R group(P <0.05). The expression of NF-κB and ICAM-1 suppressed significantly in liver and intestine tissue in rats receiving SMB compared with L.R group and SAP group. It suggests that over-activity of RAS caused by SAP in rat plays an intermediating role in splanchnic organs′ ischemia reperfusion injury and gut-original endotoxemia converting SIRS to MODS in mice. SMB resuscitation results in a significant attenuation of the SIRS of SAP by inhibiting the RAS.
