Objective To evaluate the influence of desflurane on hemodynamics and ST segment in patients with coronary
heart disease(CHD).Methods Forty CHD patients,ASA grade Ⅱ Ⅲ were randomly divided into 2 groups:inhalation with desflurane in group I and isoflurane in group Ⅱ.All patients received pethidine and scopolamine intramuscularly one hour before anesthesia and radial artery and jugular cannulation under local anesthesia,and were induced with fentanyl,etomidate and vecuronium and intubated with doppler endotracheal tube after laryngeal surface block with 2% lidocaine.Ventilation was controlled by Ohmeda Excel 80 ventilator and the concentration of desflurane was measured by Datex gas monitor and ECG was recorded by PI200A.Results No significant differences were found between the two groups in age,sex,height and bodyweight.MAP and SVR in both groups changed unsignificantly changes at 0.5 MAC,while decreased obviously at 0.75 MAC and 1.0 MAC.No sigificant changes of HR,CVR and CI in two groups occurred at each MAC.No ST segment change related to myocardial ischemia occurred by Holter.Conclusion The cardiovascular effect of desflurane is mild at the concentration lower than 1.0 MAC in CHD patients,which may keep stable hemodynamics without the occurrence of myocardial ischemia.
